Marketing agency in Guatemala for clinics, hospitals and businesses

MAD Lion Digital is a physician-led marketing agency based in Las Vegas, Nevada, with an office in Guatemala. We work with clinics, hospitals and businesses in Guatemala, in Spanish and English, and we help Guatemalan companies reach customers in the United States. Guatemala is where it all started: our founder, Dr. Alejandro Matheu, opened his first agency here in 2012.

Guatemala at a glance

A few facts shape how marketing works here, for local customers and for Guatemalans abroad:

  • Population: the 2018 census counted 14.9 million people. After adjusting for people it missed, the Instituto Nacional de Estadística (INE) estimates the 2018 population at 16.3 million and projects about 22.7 million by 2050.
  • Languages: INE notes that Guatemala has 22 Mayan languages, plus Xinka, Garífuna and Spanish.
  • Internet: about 60% of people used the internet in 2024, according to International Telecommunication Union (ITU) estimates published by the World Bank.
  • Mobile: about 20.6 million cellular connections were active in late 2025, more than the number of people, according to GSMA Intelligence data in DataReportal’s Digital 2026 report.
  • Facebook: in the same report, Meta’s ad tools showed about 9.6 million Facebook users in Guatemala in late 2025, roughly 81% of adults. That is an ad-reach estimate, not a count of active users.
  • Ties to the United States: Guatemala received about US$25.5 billion in family remittances in 2025, up from US$21.5 billion in 2024, according to Banco de Guatemala. The Migration Policy Institute counts more than 2.3 million people in the U.S. who were born in Guatemala or reported Guatemalan origin or ancestry in 2023, based on Census Bureau data.

Spanish first, phone first

For most Guatemalan audiences, campaigns start in Spanish, written the way people here speak and search, not translated from English: local names for services and neighborhoods, prices in quetzales, local phone formats. If many of your patients or customers speak K’iche’, Q’eqchi’, Kaqchikel, Mam or another Mayan language, key messages in that language, checked by a native speaker, show respect and prevent misunderstandings.

With more mobile connections than people, assume your customer is holding a phone. Pages should load quickly on mobile data, put a tap-to-call number and directions near the top, and make it easy to send a message.

Local search and WhatsApp

When someone looks for a doctor, a lab or a store nearby, Google shows businesses with a Business Profile. Complete the profile in Spanish, with the exact address including zona and municipality, hours, photos and services, and keep genuine reviews coming. Reply to every review without confirming that the reviewer is a patient. Our guides to local SEO and patient reviews explain the details.

Meta also lets businesses run Facebook and Instagram ads that open a WhatsApp chat. They suit appointment requests and quotes if someone answers quickly. Clinics should decide in advance what staff may ask in a chat and keep clinical details out of it.

Reaching Guatemalans and other customers in the United States

Your customers may live in both countries: relatives in the United States who pay for a service in Guatemala, Guatemalans planning a visit home, or buyers of Guatemalan products. Spanish matters here too. In 2023, about 72% of Guatemalan immigrants ages 5 and over spoke English less than “very well,” according to the Migration Policy Institute.

  • Your website carries the load. A Google Business Profile is only for places where you serve customers in person, and a P.O. box or mailbox doesn’t qualify, so you can’t list a U.S. location you don’t have. A bilingual website with clear prices, international phone numbers and WhatsApp fills the gap.
  • Ads can follow interest. In Google Ads, the “Presence or interest” setting can reach people who have shown interest in the places you target, not only people who are there.
  • U.S. email rules. When you email customers in the United States, follow the FTC’s CAN-SPAM requirements. Among them: identify the message as an ad, include a valid physical postal address, explain how to opt out and honor opt-outs within 10 business days.

Healthcare marketing in Guatemala: trust first

Before booking, patients want to know who will treat them. Show each doctor’s full name, specialty, training, colegiado number, languages spoken and a real photo, and explain each service in plain Spanish. Our article on social media for physicians covers how doctors build that trust online.

Health advertising has extra rules. Google restricts healthcare ads and asks advertisers to know the local laws of each location a campaign targets, so a campaign aimed at Guatemala and the U.S. must meet the requirements of both. We review claims with your medical team before they run; for legal questions, consult your attorney.
